Abstract
Clinical research has gradually revealed that the idea that there is a universal response to treatment and secondary prevention of periodontitis and peri-implantitis does not fully reflect the true clinical situation.

Patients with similar periodontal clinical conditions can show different responses to treatment and develop the disease at different rates. This requires a radical rethinking of diagnostic approaches and therapeutic choices, highlighting an individual prognostic assessment of the patient’s risk profile as well as of the tooth or individual lesion.

In complex periodontal cases, diagnosis, prognosis, and initial treatment planning are to be based on complete understanding of the patient’s individual response to the control of periodontitis and modifiable risk factors. Reassessment of the case (performed 6 to 12 weeks after completion of the first stage of periodontal treatment) makes it possible to assess the response to treatment and specify a prognosis for the case and the potential need for further surgical treatment to teeth and implants so that the treatment plan can be finalized.

Compromised teeth and implants represent a formidable clinical challenge. Advances in regeneration of periodontal intrabony defects and the parallel development of newer biomaterials and novel surgical techniques have significantly changed the prognosis of single rooted teeth with increasingly complex defects. Similar significant clinical improvements can be achieved by applying periodontal regeneration to complex intrabony defects combined with furcation involvement in both maxillary and mandibular molars.

Application of these concepts might also represent a clinical solution for the treatment of peri-implant defects. The expected clinical outcomes are reduction in pocket depth, bone gain and no bleeding on probing. These outcomes should translate into excellent tooth and implant retention during a long-term follow-up period.

Exercises will be performed on specifically designed silicone models with step by step supervision by the speaker.
